Hmm well we dont know about their first time, coz they both are pretty old, i mean like years they have been shinigami and all we know is wat happened with the present story. They may have visited Karakura before Ichigo was even born.

Well... Hitsugaya was present during the OVA, but other than that, he hasn't been to the real world during the Bleach anime. Furthermore, he was never in a gigai during the OVA.

In the opening we see Hitsugaya fighting the whale, whatever thing, but in the previous opening, he was fighting Ishida, and that never happened. I think you're talking about the manga... *can't spoil it any further*
